Job Description & Requirements Specialty: LPN / LVN Discipline: LPN / LVN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Job Title: LPN Medication Reconciliation Nurse (Remote) Job Description: The LPN Medication Reconciliation Nurse (Remote) will be responsible for reviewing and reconciling medication orders across contracted facilities' Medication Administration Records (MARs) and Electronic Medical Records (EMR) to ensure accuracy and help prevent medication errors. This is a remote position , though occasional facility visits may be required. Key Responsibilities: Complete monthly medication reconciliation of facility MARs with InnovAge medication lists Communicate medication-related discrepancies to providers, facility staff, and pharmacy partners Pull reports and conduct thorough review of orders throughout the month Accurately document all findings in the EMR and related applications Coordinate follow-up on corrections and ensure completion Conduct monthly visits to facilities as needed to obtain MARs and coordinate with staff Maintain in-depth knowledge of medical terminology, pharmaceutical names (generic/trade), and dosage forms Work independently with minimal supervision while maintaining speed and accuracy Communicate effectively with pharmacists, nurses, and physicians Required Qualifications: Active LPN license in the practicing state 3 years of clinical experience in a healthcare or long-term care setting Demonstrated clinical assessment skills Familiarity with PACE programs or similar interdisciplinary care models Knowledge of healthcare regulations, nursing practices, and medical procedures Excellent interpersonal skills and comfort working with interdisciplinary teams Proficient in planning, organizing, and executing clinical responsibilities Strong computer skills and familiarity with EMRs and email communication Ability to build positive relationships with team members, participants, and external partners About MedAdventures MedAdventures: The Best in Travel, The Best in Benefits At MedAdventures, we know that healthcare professionals deserve more than just a paycheck—they deserve adventure, opportunity, and security .
