Job Title: Registered Nurse (RN) – Long-Term Acute Care (LTAC) Department: Nursing Services Reports To: Nurse Manager / Clinical Supervisor FLSA Status: Non-Exempt Job Type: Full-time / Part-time / PRN Position Summary: The LTAC Registered Nurse provides direct and comprehensive nursing care to medically complex patients requiring extended hospitalization. The RN collaborates with interdisciplinary team members to develop and implement individualized care plans aimed at improving long-term outcomes. This role involves advanced clinical skills, critical thinking, and the ability to manage patients with chronic illness, post-surgical needs, mechanical ventilation, and wound care. Key Responsibilities: Deliver patient-centered nursing care based on the nursing process (assessment, planning, implementation, evaluation). Monitor and manage patients with complex conditions including ventilator dependency, tracheostomies, wound vacs, central lines, and long-term IV therapies. Administer medications, manage infusions, and perform nursing interventions per physician orders and established protocols. Document assessments, interventions, and responses accurately in the electronic medical record (EMR). Collaborate with physicians, therapists, dietitians, social workers, and other staff in coordinating care. Educate patients and families on care plans, discharge needs, and condition management. Maintain infection control practices and patient safety measures at all times. Respond quickly to changes in patient condition and initiate appropriate interventions. Participate in rounds, shift hand-offs, and interdisciplinary meetings. Adhere to hospital policies, procedures, and compliance guidelines. Required Qualifications: Current RN license in the state of practice (compact license accepted, if applicable). Graduate of an accredited nursing program (ADN or BSN). BLS certification required; ACLS preferred or required depending on patient acuity. Minimum 1 year of acute care or LTAC experience strongly preferred. Ventilator and trach care experience preferred. Ability to work efficiently in high-acuity, fast-paced environments. Competency in use of electronic medical records (Epic, Meditech, or Cerner experience a plus). Commitment to evidence-based practice and continuous learning. Working Conditions: May be required to work nights, weekends, holidays, and rotating shifts. Frequent standing, walking, bending, and lifting (up to 50 lbs). Exposure to infectious diseases and hazardous materials; PPE provided. Must be able to respond to emergencies and provide physical care when needed.
